What this is and why

Forbids an orchestrated lane from running above the declared share of its context window without a recorded, bounded decision about it. Every lane the fleet declares is measured at every observation cycle from the session's own usage record, busy or idle; a lane over the threshold with no live decision is a finding, as is a lane that cannot be measured, a running session the fleet never declared, and a declared lane that is not running. The threshold is an operator-declared value bound to the words it was declared in, so it cannot be moved quietly, and a recorded decision expires, so a deferral cannot become a permanent exemption.
